Catholic, pro-life mission organization seeks a highly efficient and organized team player with mental health experience to assist in the support, management and successful progression of parenting women sometimes with mental health and/or substance abuse disorders via Exodus Case Management services.

One full-time Exodus Case Manager to facilitate Exodus case management for formerly housed and supported mothers and children in Good Counsel’s four New York homes. Because of the travel to visit each transitioned mother in the greater New York Metropolitan region this new case manager will need a dependable car in good condition that Good Counsel shall provide.

Duties:

  • Conduct effective monthly one-on-one sessions with each prior resident utilizing the family development goal-setting technique in conjunction with weekly goal setting from each resident on her case load.
  • Assist women with maintenance of entitlement benefits (SSI, SNAP, Public Assistance)
  • Ensure mothers continue to coordinate consistent medical and psychiatric care for self and/or children.
  • Provide crisis management for mothers employing linkages for interventions as appropriate
  • Assist mothers in setting and achieving their individual Exodus goals.
  • Home visits – complete a minimum of one every month for the first 6 months and ongoing for the following 18 months
  • Maintain telephone and email contact with mothers who have discharged despite the reason.
  • Compile necessary statistical data to analyze the effectiveness of all internal life skills program against the reality of the residents’ ability to remain self-sufficient

This new Exodus case manager will also be responsible for acquiring data and maintaining accurate databases to track the progress of each mother and her children, in the following areas and activities:

  • Educational efforts and attainments of herself and her children;
  • Worship practices of herself and her children;
  • Significant social relationships of herself and her children;
  • Employment status;
  • Financial stability;
  • Medical status of herself and children;
  • Developmental guidance and activities of herself and children;
  • Life Skills not included above;
  • Casey; and
  • Let’s Go Learn.

This data will be used in providing more comprehensive counseling and guidance of each mother and will also be used to evaluate and refine Good Counsel’s overall Life Skills and other developmental programs both for their effectiveness in providing the most appropriate guidance and support for each Good Counsel mother but also to ensure that they have been accomplished in the most cost-effective way.
