- The Case Manager conducts the initial assessment and orients residents to the home and sets individual goals.
- This is a hands-on role. The Case Manager does everything from working with residents in the home to taking them to doctor appointments and accompanying them into the delivery room when they're giving birth.
- Ensures all residents apply for and are receiving all entitlement benefits within 45 days of coming into the home.
- Teaches personal financial management and career development to residents.
- Develops an appropriate discharge plan for each resident to ensure a smooth transition from home to independent living with available mental health support.
- Provides pro-active case management for two years to residents who move into private living.
- Compiles statistical data to analyze the effectiveness of all internal life skills programs.
- Maintains accurate records including resident files, Casey life skills data, discharge reports, weekly and incident reports, and financial snapshots.

- MUST have experience working with mentally ill and substance-dependent residents.
- Must be knowledgeable of social services resources available in the NY area.
- Has experience working with clients to set goals and holding them accountable to achieve them.
- A confident advocate for her client.
- Experienced with a variety of mental health diagnoses.
- Suggests better approaches and thrives on change.
- Admits mistakes freely and openly and is looking to build a drama-free community.
- Respects their co-workers and the families we serve as valuable individuals.
- Familiarity with Casey Life Skills Assessment is helpful.
- Enjoys a hands-on role working directly with residents including doctor's appointments to the delivery room.
Qualifications:
Job Type: Full-time
- Bachelors or Masters Degree Required in human services, social work and/or psychology.
- Extensive experience working with clients with mental illness or substance abuse challenges.
- Drivers License and clean driving record preferred.

- 20 benefit days and 8 sick days
- 1 hour paid prayer time per day.
